Your Online Connection to Personal Health Information

My AVMC Health allows you to view your medical record, as well as store, update, and share your personal health information through a secure website. Patients may also designate a representative whom is allowed to access their information. You can even download and print your records! Parents can securely access their children’s information until the child’s 12th birthday.

What is my Personal Health Record?

Your health record includes clinical data from your hospital electronic medical record, as well as your personal health history, including:

  • Lab results
  • Documents
  • Medications
  • Immunizations
  • Allergies
  • Imaging results

Existing Users

If you already have a My AVMC Health patient portal account—you may access the portal here.

New Users

New users may follow these steps to sign up for a My AVMC Health portal account. You will need your social security number, personal identifier(s), or the email address you provided during the hospital registration process in order to self-enroll in the patient portal. Start your account today!

If you provided your email address during the hospital registration process, and indicted you wanted to sign up for a My AVMC Health account, you will receive an invitation by email.

From this email:

  1. Click the Accept Invitation link in the email.
  2. Answer the security questions to verify identity, agree to the terms of use and privacy policy, and click Create Your Account.
  3. Verify your email address, create a username and password, and select your security question and answer.

Remember your username and password. You will need them every time you log onto the My AVMC Health portal.

Using the Patient Portal from Your Mobile Device

To use your mobile device, download the free HealtheLife application from the app store for iOS or Android. Then follow these easy steps:

  1. Search for the organization: Antelope Valley Medical Center
  2. Enter Username and Password
  3. Sign-in
